Tobramycin assays for once daily dosing in adults: Target range and results interpretation.

• The target range for the "Pre-dose" is <1.0 mg/L.
• Normal pre-dose level:
  • The once-daily regimen can be continued. 
  • A further "pre-dose" level should be performed following 3-4 more doses so long as renal function is stable.
• Pre-dose level is between 1-2 mg/L (and renal function unchanged)
  • The dosing interval should be increased to 36 hourly.
• Pre-dose level is >2 mg/L 
  • Further tobramycin doses should be withheld until discussed with microbiology  
  • The need for tobramycin therapy should be reviewed.
